We do not see a doctor regularly and/or we do not have the money for a doctor visit, what should we do about the required health and physical forms?

Frequently Asked Questions

This is not a problem. The statutes state that a parent may object in writing to the health requirement because of religion. You do not have to say what religion you are. You can get a form from your local health department, or you may write a letter and, sign it, upload it to your HPA student profile. You may also use our letters of exemption. Please see the link below for letters of exemption. The link provided here is good for the health examination and immunizations; you may use it for either or both.
