We do not see a doctor regularly and/or we do not have the money for a doctor visit, what should we do about the required health and physical forms?

This is not a problem. The statutes state that a parent may object in writing to the health requirement because of religion. You do not have to say what religion you are. You can get a form from your local health department, or you may write a letter and, sign it, upload it to your HPA student profile. You may also use our letters of exemption.
